How do I use the google form?

Simply open the link you receive, make a copy to your own Google account, and customize it as you wish. Then send it directly to your clients-their responses will come straight to you. 

Is the questionnaire only for Full Service Clients? 

While I created it for my full-service planning clients, you can use it with any client you're creating a wedding design for-partial-planning, design-only, or even day-of-coordination if you're helping with design and decor.
